some of the few times I've seen this happen, it was caused by, wheel spacers ( I think we can rule that one out in this case ), The wrong length lug nut shanks for the wheel, and I suppose loose lugs could do it but I don't think that's likely here. I've also seen axle flanges break off but only with stock axles, custom axles breaking when their the equal length and you switch sides with them on reassembly.Don't worry about what you can't change.
