These database errors has been happening quite often as of late.

Tyler, I had similar problems on some vB sites I manage. Usually, from the unique indexes being incomplete or a corrupt SQL dump. Shut the site completely off for about 10 minutes and run the repair/optimize tables function (for a couple of cycles) in the AdminCP on a monthly basis and it tended to solve the problem. Sometimes it's a host issue, but the template system that vB is based off of can get glitchy from time to time. Especially, when you have a massive amount of traffic and attachment multimedia in a single thread. (e.g., Random Picture of the day)
