Where can I get a Vehicle Inspection Report

The beater I am driving needs a Vehicle Inspection Report so I can apply for a Salvage title. I think I need form VTR-64.

Someone ran into the back of my car over a year ago, the damage wasn't bad enough to decommission the car, it's just not worth it to fix it. I cannot renew my registration with obtaining a Salvage title.

I just got my notice in the mail for registration renewal. It says the Vehicle Inspection Report is your proof of a passing vehicle inspection. Not sure if this what you need but it sounds like it.

No this is an inspection you need to have done after you had your totaled car fixed.
