engine cooling issue - bad radiator?

I doubt it's a head gasket. I would buy a pressure tester and make sure there are no leaks. If you cant find any check the thermostat and then water pump.
How many miles do you have in the truck? Any idea if the water pump has ever been replaced?

I can't read the post but you didn't not mention the following in the first sentence
Replace
Thermostat
Flush coolant
Verify water pump is functional by bleeding the system as it warms up. Leave cap off the radiator to ensure waterpump is functional as air is bled by the thermostat opening.
Don't over think the problem. Is coolant full and clean? Has thermostat been replaced?
