          Last resort would be small claims court but be sure to do it quickly and not let a time period pass that might not allow to file the claim.

            I've seen it go both ways. In a nutshell, it's typically covered if the driver knew of the medical condition beforehand. If it's a first time event and that medical condition was the causing factor of the accident, you have a reasonable basis for a denial. With that being said, the amount of exposure can play a key in how much an insurer wants to defend their decision.

                  His best bet may be to sue in small claims court. And hope that she pays if he wins. Don't have to worry about bullshit insurance policy exclusions there.
