I don't know, I've gone to subway and paid for a footlong and receive a 9" sandwich, it was definitely no foot long, but that's not often, most of the time it's fine. But I do share the sentiment on occasion. But, leave it to some Jersey boys to sue over a damn sandwich, they sure do love their subs. Jersey Mike's conspiracy theorists in 3..2..

Okay. Correct me if I'm wrong. But I'm pretty sure bread rises. Meaning when you bake it, it gets bigger, not smaller.
Depends entirely on the type of bread, use of yeast, and even the recipe, etc. As already stated, if you don't proof the bread properly, it can shrink. If you overrise the bread, it can shrink. A million and one things can happen if not done properly. Baking bread is almost an art. There's a fuckload of science going on when baking a loaf of bread.
