So we assume that he didn't holster his shit properly. And we know (based on the article) that she hugged him from behind. They both made mistakes, but her mistake was more directly responsble for what happened, imo.
Why would anyone "hug" a cop (or anyone wearing a gun) from behind?
According to my blame calculator, this is 67.4% her fault, 12.2% his fault, 15.3% Obama's fault, and 5.1% of the blame falls on the Texas rat snake.

Personally I don't carry as much as I used to, but for me it depends on which gun I'm carrying...My Colt and Springfield both have much safer more positive feeling safeties (beavertail and grip), so I do usually carry them hot with the safety on. However my Glock doesn't feel as safe, and I still don't fully trust that trigger safety myself...so it's not normally racked.
