Only take deposits in form of a GIFT. If they send it regular, it is refundable because they didn't get the item. Simple as that. If I do take a deposit on something through paypal and they send it regular. I simply refund it and tell them to send deposit as gift and nothing else or I won't accept.

When requesting the paypal money for the deposit i put in there in all caps that the deposit is non-refundable as stated in the ad.
So far I have had no issues.

I never pay 3% when receiving it as a gift. Thats a way my friends pay me when they don't have cash and need to buy something from me.

Forms 2 and 3 are the same. I just fill out one showing 1,000 has been paid on whatever date via paypal and show the remaining balance. Then on the 2nd one I show that the remaining has been paid via cash, wire, etc and 0 is owed.

I usually sell cars for my father in law so the money forms have 3 signatures. Mine, my father in laws, and the buyers. The forms are in Word so you can alter it however you want it.
