We were talking about this at work. Really, there had to be more than one failure for this to happen. The first is her doing, which is the most critical. After that it had to be poor prep and not watching her afterwards.
At this point though, I just can't blame the pilot. It's a sad accident, but the only person to blame is the girl. The only exception I'd make is if the pilot was moving the plane and hit her or some other negligence.

I have compassion on the level that this girl just ruined a big part of her life, and it's tragic that such an incident occurred. The pilot should have shut down the plane for passengers to off load. Perhaps he did not want to do a "Hot Start", which is sometimes problematic depending on the plane.Comment
