    A pistol is a defensive weapon and are designed to carried loaded. Follow the rules and it is as safe as a rock.

      We do drills every week on loading/reloading, and we do it timed. I can tell you that first hand, with no one trying to kill me, and in a secure environment,I can slip on the slide easily half of the time. When your adrenalin starts going, and your hands start sweating, you would be extremely lucky to actually get the round where it needs to go, and not screw something up.

      Putting a round in the chamber is very easy when your hanging out at the range. Now try to do it when you only have 2 seconds to rack it, aim, and squeeze in the right direction.

                Round chambered wasn't the problem. Guy improperly holstering and controlling a firearm was the problem.

                Further, when the SHTF, your motor skills go to 50% of the highest level you trained at. Racking a round is the last thing you need to worry about. Also, more than likely you're reacting to something happening to you, so every second counts.

                  I've never understood how anyone with inelegance higher than your average monkey walking the streets could think a chambered gun is a good thing to put in your waist band. You deserve a bullet to the nuts for that. My dick is worth the $50 I spent on a holster.

                    I never thought of monkeys as elegant I guess. Also, all guns are chambered in one caliber or another.

                        Carrying on condition 3 is great if you will have the drop on your assailant but then again if you have the drop on them you have the ability to avoid the situation all together. When confronted by a man of low moral character intent on doing you harm you will have barely time to clear leather and fire much less time to load your firearm. Remember boys and girls there are no time out's or do over's. Train the way you fight and you will fight the way you train.

                        Personally I carry either a j-frame 38 with 5 rds loaded or a 1911 cocked and locked with 8 rds loaded (yes with one in the pipe) and always in some form of a holster be it a pocket sheath or a IWB
