If it's destroyed, it could only be replaced under insurance that you might carry. If it's defective and is being replaced under warranty, you can get S-on w/ the same utility to root and S-off then run the stock RUU to return it to original "as shipped condition".
As of now, there's no way to tell anything has been done. Besides, if you're sending it in you're gong to want ALL your info off of it and the RUU does exactly that.

I know for fact it sill works. Ma67cpe used it, I've used it on several Evo's an a good friend of mine used it to do one today. It's dang near as simple as UR3.
