I would have to start a whole new thread for IT disasters that I've had to go in and fix. My favorite was the wire splice that a guy did to the main internet feed by using 8 individual snap connectors to splice two 3 ft cat5 cables together.
He didn't want to spend money on a network cable "he didn't need".
Sucks for him - he coulda bought a $5 cable, but he spent $130 + a $10 cable for me to come out and fix it.

Our replacement rate on those cables over time is...well I think I've seen it done maybe 5-10 times. It's been an evolution since we started doing them that way in 2006 (far less cables), but needless to say 130k servers later we must be doing something right. To be fair that is a super dense row. Our typical rows are a bit less cabling.
