Catastrophic engine failure on locomotive

    Amazing! Worst engine failure carnage ive seen was during Desert storm. Jet took off and climbing out thru 10k when the tail engine chunked a fan blade, causing the whole thing to fly apart, blew off both fan cowl, thrust reversers. huge holes in both horizontal stabs. They declared and emergency, dumped fuel and landed, but what a mess. Took 4 months to fly in support equipment and make the repairs. Same failure as United 232 had that ended up crashing in Iowa. Due to a tcto that installed a #2 engine firewall shutoff valve, this airplane only lost the #2 hydraulic system, not all 3 like UAL 232.

            For the entire power cylinder (liner, head, injector, etc) AND the piston to be ejected bad stuff happened.
            Note that there's no rod on that piston. Makes you wonder what broke first.


            Just a guess: Power cylinder not properly secured, comes loose, gets launched via compression, piston has no liner (to guide it) and no compression to counter the upward speed, snaps wrist pin, follows path of the jug??

            I posted those same pics on the other site years ago and could never find an official cause of failure.
