I got one dead center of my foot when I was a kid. Dug it out with a pocket knife. Hurt like fuck, but I was sick of it catching the carpet and my sock.
I understand that you're a doctor, but I went to the doctors office a couple of times for some shit on my pinky toe and they kept saying they're "corns" and to just use the Dr. Scholl's stuff and it never worked. Finally I was sent to a specialist...this old dude that LOOKED crazy but when he first saw it and looked at the folder from my usual doc was like "Yep....they're warts." He prescribed me some cream, and after a couple of months of applying it every day they're gone and haven't been back in nearly 15 years. However, it was on the top of my toe...not the bottom.

go to the podiatrist, I had a spot on each foot that looked exactly like that. The doc said that the pain could be attributed to blocking sweat glands. He cut them off with a scalpel 5 years ago, I bought different shoes and haven't looked back.
I walked around work for like two years with pretty intense pain and it took him all of three minutes to fix my feet.
