I went to a party last night at an apt club house that had a big tv over an outside fireplace. It looked pretty cool until you realized that it was warped from the heat of the fire.
As a home inspector I see many televisions mounted over fireplaces. The biggest problem is that people will run cable and electrical wiring down the fireplace chase which is against electrical code. If your home ever had a fire and the fire investigator found this condition, your home insurance provider would have a way out of paying out a claim.
Also, if you have a problem with your television you should never tell them you had it mounted over a fireplace. Most warranties do not cover placement over a fireplace cause they are prone to heat damages from the firebox. (Plasma's are the worst to mount over a fireplace for this reason)
it doesn't matter how good of a deal you got on geek squad, it's not worth it. Hopefully you at least saved $75 and bought the mount from monoprice.com. The install is SUPER easy... you just need a level, tape, drill... and a screwdriver. I installed mine in about an hr, and I was taking my sweet ass time to make sure it was level.
This. My buddy put his 58" plasma at eye level while standing. Tad to high when sitting to watch it. I think it diminishes the picture quality a little

Slightly tilt the top of the unit forward, that will help the visual clarity.

That looks too high for comfortable viewing. It looks great on the wall, but to actually watch it on a daily basis looks painful. We get requests for this type of install quite a bit(not always above a fireplace, but the same height). Normally we use masking tape to show where it will be, then have the customer sit down and make sure they're comfortable. If they still want it that high, we'll do it. Most go with a lower height after the tape demo.
My preference is for the center of the screen to be just barely higher than eye level when sitting. Too high of a mount makes it feel like a commerical location(bus station, airport, sports bar).

Aside from being exposed to extreme heat or direct flame, heat damage is hard to identify regarding warranty coverage. I would give the customer the benefit of the doubt, but also let them know that heat is the probable cause of the failure and recommend they relocate the unit. Also LCDs are more sensitive to heat than plasmas. Plasmas generally have more heat sinks/fans internally to help shed heat.

The 103" we mounted is just high enough so the customer doesn't see their feet when reclined.
